News that VNSNY Care360° Solutions has partnered with the Chinese Community Accountable Care Organization (CCACO) was featured in the February 25 edition of Crain’s New York Business.
The partnership marks VNSNY Care360° Solutions’s first with an accountable care organization. In this arrangement, VNSNY Care360° will provide care management services—including initial in-home assessments, home care monitoring and interventions from clinicians, and telemedicine to track vitals and improve medication adherence—to up to 400 patients, with the goals of reducing hospitalization rates, ED use, and costs of care.
In their article, Crain’s notes that there will be significant focus on addressing social determinants of health, and that “the ACO and VNSNY have worked side by side in using data to determine the highest-risk patients,” and that they’ll continue to use data during the next 12 to 18 months to track the outcomes of providing services.
